Payment Options
Credit Card / ACH: You will be asked for information from your invoice/statement, so have it available. Any convenience fee that applies is shown on the secure payment page before you confirm.
Check / Money Order: Mail to the address in the Contact section.

Insurance Information
Please check your invoice/statement for an IMPORTANT MESSAGE section at the top. If we do not have your insurance information, or need a signature, a request was included with your statement. If there is other insurance that can be billed, download and complete the insurance information form and mail, fax, or email a scanned copy to the billing department.
